<div>For individuals with a history of COVID infection, the following patient groups should be routinely assessed for mental health concerns and mobility, functional, swallow, and/or cognitive impairments to determine discharge readiness, rehabilitation, and follow-up requirements:</div> <ul> <li>patients that are in or have been discharged from intensive care;&nbsp;</li> <li>older patients that have experienced severe cases; and&nbsp;</li> <li>patients that exhibit signs of any of these impairments (GREEN).</li> </ul>

<div>Patients with a history of COVID should be provided with tailored rehabilitation programs from the post-acute stage to long term based on their individual needs (GREEN). The patient should have access to professionals, which may include speech-language pathologists, whose skills align with the patient's needs (GREEN).&nbsp;</div>

<div>Ensure multidisciplinary collaboration of necessary healthcare professionals in the decision-making process to address multiple morbidities and functional decline of older patients with COVID-19 (GREEN).</div>
