<div>Evidence regarding aspiration risk in adults with cuff-inflated tracheostomies is inconclusive. None of the examined studies demonstrated a significantly higher incidence of overall aspiration; however, the risk of silent aspiration was higher in individuals with inflated tracheostomy cuffs. Given the heterogeneity of assessment methods, patient populations and methodological quality of included studies, further carefully designed research is warranted.</div>
