Superior accuracy was noted for the ADOS-2 when compared to the ADI-R for use in clinical settings with children suspected of being on the autism spectrum. ADOS-2 sensitivity ranged from 0.89-0.92, while its specificity ranged from 0.81-0.85. Mixed results were noted when comparing the accuracy of the ADOS-2 in clinical versus research settings, however differences in accuracy between settings were generally not significant. ADI-R sensitivity was noted to be 0.75, while its specificity was 0.82. Specificity for the ADI-R was reduced in clinical (0.72) versus research (0.85) settings, however sensitivity did not differ between settings. Limitations to this review include heterogeneity between studies and significant risk of bias regarding patient selection and reference standards within studies.
