Deriving Clinical Meaning From a Medical Chart Review

The
efficient, organized, and thoughtful collection of a patient’s medical history
is a critical first step in creating a comprehensive care plan that includes
meaningful assessment, appropriate recommendations, and evidence-based
treatments. Your practice is likely filled with patients whose medication
effects, cognitive impairments, physical impairments, communication challenges,
and other characteristics interact in unique ways, all of which you must
consider as you put together a plan of care. In this course, Martin Brodsky
will introduce common pitfalls of medical records and charting, with specific
focus on areas of the medical record that are relevant to speech-language
pathology. You’ll walk away with a list of resources that are free, readily
available, and quickly accessible as well as strategies to overhaul medical
history interviews from feeling like interrogations to bringing about
resolutions.
After completing this session, you will be able to:
- explain the goals for medical chart review
- identify which areas of the medical chart are needed to inform speech-language pathology diagnosis and treatment goals
- employ at least two pragmatic and time-saving tips to assist with effective and efficient navigation of the medical setting
